QIA Example  reflection (Case review)   - Domain Two Safety and quality (Respond to risks to safety)

I think the first time anyone of us saw a patient with Covid symptoms other than being concerned for our own safety, there were a couple of things that struck us. How best could we manage the individual patient but also protect ourselves and other patients? It took discussion at regular meetings (daily when everything first kicked off) of four or five patients that had presented acutely unwell to the surgery, to lead us to introducing some structural changes that improved care for individual patients and protected others. This resulted in us initially dividing the practice up into hot and cold areas and subsequently adapting an outside area for suspected Covid patients. Each initial patient informed the development of this plan.

Our appointment system has always been a challenge. Not only for patients trying to get into see us but also for receptionists that have a multitude of slots that they can fill.

Over the course of a few days the changes that we introduced into the surgery to stop people coming for appointments were incredible. We initially phoned all patients that had prebooked slots and subsequently only offered telephone appointments. These appointments were tweaked to form a Hot and Cold triage list which every doctor picked off This has now reverted to a same day telephone surgery for each Doctor that is in surgery with provision for rooms for those patients that needed to be seen. We are currently revamping this and are developing further information which can be hosted on the practice website which is where we have largely been notifying patients of any changes.
