What do I need to include?

Personal & Professional details – you will need to ensure you keep your details up to date. Each year as part of the annual declarations you will need to confirm that you have checked and/or updated your personal & professional details.

When: ANNUALLY

With the types of supporting information below, you should be focused on reflection, learning and outcomes. 

CPD / Appraisal information – there is no set minimum or maximum for the quantity of information required for appraisal, GMC guidance states that ‘during your annual appraisals, you will use supporting information to demonstrate that you are continuing to meet the principles and values set out in Good Medical Practice do not recommend any particular number of hours of CPD, however there must be sufficient material on which to base a meaningful appraisal. The emphasis should be on quality over quantity. Further information on this can be found here. You should ensure your CPD covers the whole scope of your practice.

When: ANNUALLY

Review of complaints & compliments – You must declare all formal complaints during the appraisal period. These complaints may include as yet unresolved complaints or complaints from a previous period that have been resolved during the current appraisal period. Minute details of the complaint are not required, your appraiser will be interested in the fact you are engaging in the complaint process and any learning outcomes or changes made as a result.

Compliments are an important part of feedback and should be included as such. When: ANNUALLY

Significant Events / Critical event analysis – you should document any unintended or unexpected incident which did or could have led to harm of one of more patients. Even if you were not involved directly in the event but have learnt or changed your practice as a result, you can include this.

When: ANNUALLY

Quality Improvement Activity – This should be relevant to your work and include an element of evaluation & action.

When: ONCE PER REVALIDATION CYCLE

Patient Feedback – this is currently facilitated by Orbit360, you may also hear this referred to as ‘360 Feedback’ or ‘Multi-Source Feedback’ (MSF). Your patient feedback should cover the full scope of your practice. Every doctor who holds a prescribed connection to an NHS designated body in Wales can register on Orbit360 and request a survey. Where possible, information from MARS and the GMC is used to authorise these however where not, these are then reviewed manually be a doctor’s designated body. If you do not see patients it may be appropriate for you to only complete colleague feedback but this must be discussed with your Responsible Officer, you can contact them through the revalidation team at your designated body.

When: ONCE PER REVALIDATION CYCLE 

Colleague Feedback – this is currently facilitated by Orbit360, you may also hear this referred to as ‘360 Feedback’ or ‘Multi-Source Feedback’ (MSF). Your patient feedback should cover the full scope of your practice and can include both clinical and non-clinical staff. Every doctor who holds a prescribed connection to an NHS designated body in Wales can register on Orbit360 and request a survey. Where possible, information from MARS and the GMC is used to authorise these however where not, these are then reviewed manually be a doctor’s designated body.
