How to report
There are many ways to report ADR to the Yellow Card scheme.
You can report directly through the Yellow Card website
You can fill out the yellow card form from the BNF/Mimms and post it back (FREEPOST YELLOW CARD).
There is also a free Yellow Card app you can download and use.

Reporting through the Vision
Vision allows you to report ADR to Yellow Card scheme directly.
It is not complicated but requires a few steps.
Step 1: click on Add: Drug Allergy/Adverse reaction
Step 2: Once you have filled in the details of the Drug Allergy/Adverse effect, click on the Yellow Card tab. This will expand the data set to fill out. Once you have filled it out click on the Send Yellow Card tab.
A new screen will appear which details the other medications also taken by the patient and reactions experienced linked to previous information provided in Vision. You can add to this screen if there are further relevant details (eg over the counter medication, other reactions, free text details etc). Once completed click on the Send button at the bottom of the screen.

The application will ask you if you want to send the Yellow Card to MHRA – click Yes to send it.
If successful, you will be notified it has been sent and a Yellow Card notification will come up in the patient’s notes.
Unfortunately, there is no direct link through Microtest at present.
