Vivian is a 67-year-old lady who lives with her wife, Ellen. Having transitioned over a decade ago, she considers herself to be a woman with a trans history. She underwent genital surgery involving orchiectomy, so for bone protection and to maintain gynaecoid fat distribution, she uses a low dose estradiol patch.

She is newly registered at your surgery and for some reason the HRT was not included in her repeat medication request. 

She comes to see you to resolve this matter, but the consultation gets off to a terrible start because she was misgendered (referred to as a gender she does not identify with) when booking her appointment on the phone.
